Step 1: Complete the required forms

Download and complete the appropriate registration forms and bring them to the surgery for submission:

Alternatively, you can collect a copy from our reception or register online via the link below.

Along with the registration form, you will receive health questionnaires that need to be completed. If you need assistance, our reception staff will be happy to help you fill out the forms.

Step 2: Provide proof of ID and address

Please bring:

  • Photo ID (passport, driving license, or birth certificate)
  • Proof of Address (bank statement or utility bill, dated within the last 3 months)
If you are registering a child under 6, please bring their red book (Personal Child Health Record) or immunisation history from previous records. This will allow us to check their immunisation status.

These documents are needed to verify your identity and to grant online access to services.

Step 3: Finalising your registration

After we receive your completed forms, we will add you to our system. Once the registration process is finished, NHS England will transfer your medical records to our practice and send you a confirmation of your registration.

You do not need to inform your previous practice of your move, although it may be courteous to do so.

We will never ask for proof of immigration status or share your information with immigration authorities. Our aim is to provide healthcare to all, especially those who may feel vulnerable or face barriers when accessing medical services.
